Compounded semaglutide from $129 and tirzepatide from $169 for the first month, with a Health Coach, a dietitian consult and free delivery included, but the rate after month one is not stated, the money-back guarantee carries strict conditions and cancelling takes a phone call.
Scored 6.3/10 · ranked #44 of 54 · $129 first-month offer (later months not stated) · Verified 2026-11-06Pick
What that price doesn’t say: $129 is a first-month welcome offer on the homepage (semaglutide; tirzepatide from $169), shown against a crossed-out $249. LevelsRx does not state what month two costs, and its own articles describe $129 as a starting monthly price rather than a first-month offer. Ask for the renewal amount before paying. Read 6 November 2026. Price read 2026-11-06.
Short answer
LevelsRx publishes $129 first month (later months not stated) as its starting rate.

The downsides we found
- The advertised price is an introductory or promotional rate, not a standing one.
- The money-back guarantee is narrower in the terms than in the ad.
What the score rests on
Classified as first-month-rises. See how we classify every clinic's price →
| Criterion | Weight | Score | What it rests on |
|---|---|---|---|
| Price & value | 25% weight | 5.5 | $129 is a first-month welcome offer on the homepage, shown against a crossed-out $249. The rate after month one is not stated on any page we read, so the price is scored as an entry price, level with other clinics that publish only a first-month figure. |
| Clinical oversight | 30% weight | 7.5 | Names three clinicians and four compounding pharmacies with addresses, and includes a Health Coach, a dietitian consult and 24/7 messaging. The medical group is named three different ways and no licence numbers are given. |
| Treatment options | 20% weight | 7.0 | Compounded semaglutide and tirzepatide injections, with coaching and a dietitian consult built into the plan. No brand-name drugs, and oral semaglutide is mentioned only in its articles, not on the plan pages. |
| Transparency & trust | 25% weight | 5.0 | The homepage promises a money-back guarantee; the Terms limit it to medication fees and require near-perfect adherence. The $129 is a first-month offer on the homepage but a starting monthly price in its own articles, the renewal amount is not given, cancelling takes a phone call, and Trustpilot reads 2.4 on 10 reviews. |
4 of 4 criteria have a written source.
Watch out for
(1) THE PRICE AFTER MONTH ONE IS NOT STATED. The homepage shows $129 as "your first month" against a crossed-out $249 and a countdown. LevelsRx's own articles call $129 "a starting monthly price, not a first-month-only offer". No page gives the renewal amount; the Terms say a promotional subscription renews at "the then-current Subscription Fee". (2) THE MONEY-BACK GUARANTEE IS NARROW. The homepage says "Lose weight with our program or get your money back." The Terms refund only the GLP-1 medication fees, over 6 months, if you lose less than 20% of your starting weight, and only if you attend at least 80% of weekly physician consultations, follow your dietitian meal plan with 100% adherence, take the medication as directed and send a scale photo by the 28th every month. Coaching fees are not refunded. (3) CANCELLING NEEDS A PHONE CALL. By phone only, not email, chat or the website, at least 7 days before your next billing date. Fees are otherwise non-refundable, including when you cancel mid-cycle, and dispensed medication cannot be returned. (4) WHO TREATS YOU IS NAMED THREE WAYS. The Terms say LevelsRx provides no medical services and connects you with independent medical groups; the assessment names OpenLoop Health clinicians and Usable Health, PC; its footer names HealthPrime Holding LLC as the platform. (5) NO STATE LIST. It serves "most U.S. states" and confirms yours in the assessment before you pay. (6) ITS OWN NUMBERS. Member results are self-reported and not from trials, some testimonials are from paid endorsers, and "250,000+ patients" is its own count. Trustpilot reads 2.4 on 10 reviews (6 November 2026) and the company has not replied to negative reviews.

What we found
The ongoing rate is not published
LevelsRx publishes a first-month price and not the one after it
Homepage, FAQ, Terms (billing and promotions sections) and the three articles read; no page gives the monthly amount after the first month. The assessment shows the offer price on its first screens; the checkout was not entered. So what you pay from month two is unknown before you sign up.
